Neither were many aware of the fact, or informed by poll workers, that they could fill out a provisional ballot to correct this problem.

I had to laugh when I read this. Did you hear about the 20 or so voters who were informed by poll workers that the funny pens they were given were special pens? They used magic invisible ink that the counting machines would be able to read. They were stylus left over from the electronic voting machines. Workers were puzzled by a stack of ballots with no votes on them and tracked down the story.
