You’re absolutely right - any such questionaire would be worthless, as the SS teachers would lie.
George Lakoff, their prophet, lays out how to handle such things, and it is basically to lie and to “reframe” the issue in SS terms instead of traditional terms.
I am sorry to tell you that you can’t count on the honesty of the teachers. My church elder’s special needs daughter was in the San Francisco public school system. They “opted out” of the sex ed stuff. (Sex ed in SF is really over the top, with visiting homos explaining their favorite activities, etc.). She went to every sex ed, every year. They simply didn’t honor the “opt out” form.
