The first was during the 2000 Olympics by Osama bin Laden. Curious note that Al Qieda tried to use this "dirty bomb" technique in Sydney before our Sept. 11 attacks and you seldom here about it. LINK

BTTT, this info needs a few more bumps. Too many on FR are not paying attention as terrorists attempt to blow up nuclear reactors.
