This was no accident, but 1st degree negligence. He broke every safety rule in the book.
It would have been a miracle if he had survived (i.e. the gun didn't fire).

Yep. Gun safety is not rocket science, but it does require attention. I am fairly new to gun ownership (only a couple of years) but I was very impressed by the directness and simplicity of the gun safety course I took.
If you follow the rules, you cannot hurt yourself or others.
