First President to take oath on Bible? (history question)

Does anyone here know who was the first to take an oath of office with his hand on the Bible?
Is it a something that congress voted in favor of at some time? How and when did the practice begin for all of congress?
I'm on two other bi-partisan political forums and would appreciate some info to respond to those types of questions.
