Heard from Shep Smith just now. Witnesses smelled gas at least 2 dead.

Four piani = four floors. It says one of the victims was Albanian and the other a 69-year-old man.

Why would the 2nd or 3rd floor in Italy be the 3rd or 4th in America?.....
Because the first floor to us is the ground floor to them (and most Europeans). The second floor to us is the first floor to them. I don't know why.

MILAN, Italy A 4-story building collapsed late Monday, killing at least two people and injuring several dozen, police and Italian news reports said.
Police in Milan confirmed a building had collapsed on the eastern edge of Italy's financial capital, but no one was immediately available to confirm the number of casualties. The ANSA agency quoted police officials at the scene as saying two people had been killed.
Several emergency and rescue officials were at the scene of the collapse, and ANSA quoted witnesses as saying they had smelled gas shortly before the explosion, which happened about 8 p.m. (1800 GMT).

I think that means 4-5 floors, not 45. There aren't many Italian buildings that are 45 stories tall.

I've been involved in the investigation of numerous fires, including natural gas explosions, and the damage in the photos looks like a classic NG explosion. Ignition of the gas blew the walls out nearest the leak (that, or just the windows and doors if it's a smaller explosion, are the weakest points where the combustion gases will escape).
Sometimes you get a complete collapse as a result of the removal of the lateral support, but in this case it looks like the building was strong and redundant enough to survive the blowout.
