Correct protocol, for years, was to refer to ex-Presidents by their previous highest title. Remember the references to General Eisenhower?

That's what I thought, but I wasn't sure where I had read it. Wasn't there also some exception to the rule based on whether the highest prior office was unique, or something like that?
