If said type of accident is only expected to occur every 100 million flight hours, then retrofitting hundreds of aircraft is considered more expensive than the loss of one now and then.
And don't think the government will mandate it. In 1972, after an American Airlines DC-10-10 lost its rear cargo door over Canada, the FAA should have ordered McDonnell Douglas to redesign the cargo door and fix all aircraft in service. Instead, they simply issued a recommendation which meant nothing happened.
Then on March 3, 1974 a Turkish Airlines DC-10-10 lost its rear cargo door after takeoff from Orly. The aircraft crashed killing all 346 people onboard.
One aspect of the conspiracy theory I’ve never seen discussed is Boeing’s involvement in the caper. Boeing would probably want to say something if a trumped up design flaw was being blamed for the loss of several hundred people’s lives. The potential impact in sales of future aircraft not to mention design costs to retrofit old aircraft with new safety systems would cost Boeing millions. You figure the good folks at Boeing wouldn't’t remain silent without hefty compensation.
I watched the whitewash on CNN the other night. I was struck at how Kalstrom and the NTSB chief continuously referred to Salinger as a nut, a derranged man, an idiot. This is not how highly educated, uncorrupted men speak.,,,unless they are trying to hide something. The aircraft was shot down, and they covered it up. Kalstrom and his cohort protested too much...they gave themselves away. They gave too many nonanswer, answers. Answered with too many questions. They were liars and deceivers.
Go back and read what I posted in #45. Sitting down there and waiting for the right opportunity to shoot down a passenger aircraft is almost the equivalent of sitting in your back yard waiting for a duck to fly overhead on its migratory route at a height of 500 feet. Anyone who intends to shoot a duck is going to plant himself close to a body of water where ducks flying overhead are going to be well within the range of the gun he's using.

How did LBJ keep an entire carrier crew as well as the crew of the U.S.S. Liberty quiet when he ordered the carrier's commanding officer not to send interceptors to protect the Liberty while unmarked Israeli aircraft were bombing it and strafing the lifeboats?

The first court of inquiry was ordered by Adm. John S. McCain Jr.—father of Sen. John McCain, R-Ariz.—who was then commanding in London and over the vessels in the Mediterranean. Kidd and Boston conducted the inquiry in Malta where the Liberty had been taken for repairs and care of the wounded. Though they both agreed the attack was intentional, Kidd prepared a report that went with the Israeli version.
"I know from personal conversations I had with Admiral Kidd that President Lyndon Johnson and Secretary of Defense (Robert) McNamara ordered him to conclude the attack was a case of 'mistaken identity,'" Boston's affidavit said.
There are numerous discrepancies between the Israeli version and the U.S. sailors' report. The U.S. sailors reported they were under aerial surveillance by Israeli planes for hours before the attack and were flying the American flag. They said they were hit by aircraft, torpedoed and strafed by fast-moving surface vessels.
The Israelis said only aircraft were used in the attack. The sailors said that when they tried to launch lifeboats to secure the wounded the torpedo boats strafed and sank them.
They reported that the Israelis jammed the radio so they couldn't call for help, but later when American aircraft were launched to help, they were recalled. Moorer said he has interviewed officers on the USS Saratoga, where the planes were based, who said the White House ordered them to abandon the rescue.
The Liberty was a World War II freighter converted to the role of a spy ship that the Navy said was very distinctive with massive aerials and satellite dishes. These spy vessels and spy flights later were replaced by satellite surveillance.
Why the Israelis would want to attack an American vessel has never been explained, but the incident came a few months before the 1967 Middle East war in which Egypt and Israel were adversaries.
That fuel tank story was almost as full of ---t as James Kahlstrom is.
Your first clue was that the FBI and CIA took over the investigation.
To my knowledge an investigation has never been wrestled away from the NTSB before.
The CIA actually had an agent on the platform for each press conference. When the NTSB did make statements, Kahlstrom (FBI) and the CIA agent were quick to interject if the NTSB guy didn't say things just right.
This investigation... scratch that, cover up was terrible.

I'm an EE and come out of A&D. Every failure is analyzed by a failure review board and alerts and corrective action issued. Even if an airline operator chose to ignore the warning, there would be a paper trail on this problem and the serial number and date of manufacture of every aircraft affected.
It's also more complicated that the loss of an aircraft considering the collateral damage that can occur in the event of a catastrophic failure.
Problems like the one you describe in the 70s are exactly why they procedures are the way they 3 and 4 decades later.
Again, is it possible? yes, but it would be very easy to prove. Here's the FRB report. Here's the alert. Here's the corrective action/ inspection recommendations. Here's the aircraft affected, etc., etc. And, they would have to explain why they ignored it especially in light of 2 other catastrophic failures.
Sorry, my tin hat is buzzing on this one... I doubt sincerely it was a Navy missile but I wouldn't be shocked to learn it was a Stinger or similar system in the hands of the Islamo-fascists.

IIRC, the US Navy initially denied being in the area at the time. They later admitted to being in the area. I don't know whether this was more or less than 100 miles.

Would that be the same Boeing that got a large government contract shortly thereafter?
They could easily have gotten their compensation, too. The U.S. government lets billions of dollars worth of military contracts every year, and would be in a perfect position to award a pile of these to Boeing in exchange for its cooperation.

IIRC, there was a study completed a couple of years later. The study concluded that this type of explosion would happen about once in 20 years and that it was not beneficial to modify fuel tank design.

Nonsense. I live and fish from my boat right around Logan. That's exactly what you'd do. Unless you know the area and waters well you'd get far away and progressively move in until you got a lock. You have no idea what height flights are a given distance from the airport - it varies like crazy and there are planes coming by every few minutes.
