Unfortunately, the control room is in the hands of whoever's providing the pool feed, and apparently they chose to put it on the line early. Most of the nets were apparently staffed with people who were savvy enough not to punch it up until the appointed time. This being the kind of thing that happens regularly with remote feeds, it's hard to conceive of anyone who saw the feed come up early punching it up because they didn't recognize there was a problem.

You check levels on preview, you don't feed anything down the line you don't want to see some idiot in the pool put on the air. At least, that's the philosophy we had during my network time.

The reality is CNN's early cut to live air was and unprofessional stunt on CNN's part... and not the White House being unprofessional, naive yes of the sleaziness of the CNN, but not "unprofessional"
