Manic euphoria can be hard to distinguish from schizophrenic delusions/hallucinations. E.g. no doubt many people briefly wonder what would happen if they were on a plane and yelled "I have a bomb". Normal people don't actually consider doing it, because they know and care what the consequences would be; a bipolar person in a manic-euphoric state may get so enthusiastic about the idea that they'd go ahead and do it, without giving any thought to the consequences (the way many manic-depressives go on euphoric shopping-spending sprees, unconcerned about the financial ruin that will clearly result); schizophrenics may quickly go from reading or seeing news about terrorist bombers to believing they actually are one, and yell "I have a bomb" because they really mean it. Unfortunately, psychiatrists usually have so little time with patients, that they often end up relying on reports from family members (or worse yet, on self-reporting), and this man's wife doesn't sound like she's quite all there either.
