I don't know about the others, but I know cable companies do monitor.

Digital cable (the stuff that is decoded by "a box") can be monitored, but I don't know if the analog stuff can be.

We have both, but don't use the digital side - I leave our digital cable box tuned to a Bigband Jazz Music channel.

In the UK I would not think there is any analogue cable or satellite still around.
