Question: Let's say it was proven 100% that the guy in the photo and the president-elect of Iran is the same guy .....then what? As in what can we seriously do about it? What (viable) options are available?

Ahmadinejads message to Iranian voters was simple. Being a self-proclaimed fundamentalist he sought a return to the moral purity of the early years of the Islamic revolution (1979-1988), has also insisted freedom in Iran is already beyond what could be imagined. His campaign took up the pocketbook issues. Iran is facing a massive unemployment (about 30%). He promised to improve the economy, create jobs, etc. In contrast to that, Rafsanjani talked more about ideology and about the deterioration of relationship between Iran and the West.
Iran's election appear to have been a slap in the face to the USA. Put a terrorist hostage taker up as their President. Criminy.
Well, that's just peachy and all....
..but even though we know what this guy did, what in the world can we DO about it? Complain vociferously? Demand he be extradited?
For those who answer "Bomb the living (expletive) outta them", consider: Russia has people there helping them with their nuke "power" program. An attack on Iran COULD be construed as an attack on Russia...who still has nukes and a bunch o' missles to deliver them on our heads.
