I would imagine alcohol was involved.
I see the surgery took 45 minutes. How long can it take to put one foot on his mouth, one on his forehead, grasp the knife firmly in both hands, pull real hard, and hope nothing bad happens? If it does, you can always say, "Well, we tried!"

Most of those headlines do not have photo accompaniment. I can be reasonably assured that when I see the headline "Toddler dies, locked in car" there won't be a photo showing the red face of a dead baby or when it says, "body pulled from river" there's not a photo of a translucent, bloated corpse in a boat or tangled in low-hanging trees and rocks.

I think the "friend" is a coward. He plunged the knife in while Arthur was asleep. When Arthur awoke the friend was not there (probably ran). If I were him I'd worry about Arthur.
