Underground testing is old, but sealing all radiation and/or hints of a test preparation isn't possible. Even when India tested their nukes, they escaped unnoticed simply because they somehow(?) got hold of US satellite trajectories, and performed the test at a time when no surveillance satellite was overhead.
I don't think there will be even a moment of time without some American surveillence equipment monitoring North Korea.

The test itself was detected right away.
The Indians aren't stupid, and they're technically very adept. It would be easy for them to identify which sunsych satellites were our spy birds. The only way you can really deal with that stuff is to either keep shifting your flyover times (very expensive in terms of propellant) or to have so many flying over that it does them no good to hide from any given satellite.

Which, of course, led the Indians to avoid the obvious signs of detection.
