The possiblity could be that the child was born "hermaphrodite". Many years ago these children grew up and became carny fodder. With the advent of plastic surgery, drs began adjusting the malformed genitals to the one that they "thought" seemed to be predominant. However, it was the luck of the draw. They did these surgeries when they were babies. They would sometimes cut off what was the makings of a possible penis and the child would eventually grow up but "feel" male and vice versa.
Recent medical advice in these cases has been to let the genitals remain ambiguous until the actual gender is more determined. Some people have waited until puberty. That could have happened in this case. However, she doesn't really specify it. I'm going by her comment about "birth defect".
If this is what was really going on, my disagreement would be on subjecting the child to the change in the same school. They should have moved to a new community. Drs even advise parents to give the children ambiguous names so that they can keep the same name...Bobbie/Bobby, etc.
I'm no expert. I just remember this from one of those medical shows on Discovery Channel.

Since this is Massachusetts, the parents could be a man and a woman, two women, or two men.
If either of the latter two choices, then the conclusion is obvious.

It does not seem to be a causative factor. We should remember the case of David Reimers, subject of the book "As Nature Made Him". David was the victim of the botched circumcision, and attempt to reassign him as a girl, because it was easier to construct female genital structure than to rebuild the male genitals.
The family, on the advice of the physicians tried to push David into the feminine gender. However, it did not work. David rebelled from the start, insisting he was really a boy. In early adolescence, David reverted back to male.
What this shows is: Nurture will not change a child's gender identity. The child will rebel, sooner or later.
Personally, I believe efforts to force a child to change their gender identity will not work. Furthermore, the harder the child is pushed, the more rebellious they will become.

If these children were raised in normal households that recognized that sometimes kids like non-girl/non-boy stuff and left it at that, instead of trying to push a freaky agenda on them, the kids would be fine... But these liberal parents want gay/transgender kids so bad because it's "cool" to them, that they fill the innocent kids heads full of sick ideas that they wouldn't have thought of on their own...

I doon't think that is the case. I did some reading about the case, and found that the parents took the child to pediatric psychologists and psychiatrists in an attempt to reverse the gender identity problem. It was only after an extensive clinical workup, that the parents took the advice of the psychologists and psychiatrists, and decided to let the child transition.
Trying to force a child into a gender role does not work. If a child has a gender identity that is in agreement with their phenotypic sex, trying to force them into the opposite gender role will not work. The child will rebel. Reference to the the David Reimers case.
However, if a child has a gender identity that is in disagreement with their phenotypic sex, no amount of therapy or pressure is going to be successful in bringing their gender identity into agreement with their phenotypic sex.

No, it is a well documented medical phenomenom. History has recorded the condition in the Greek and Roman eras. The condition exists in other cultures, such as the Hajira in India. In fact, the condition existed in the American Indians, the berdache.
Animal research has shown that the use of androgen blocking agents during pregnancy can produce offspring with reversed sexual behavior. A number of years ago, pregnant rats were treated with cyproterone acetate, a potent androgen blocking agent. If the Cyproterone acetate was administered at the appropriate time during gestation, male rat offspring would be produced that had sex reversed mating behavior.
I would suggest the same thing occurs in humans. It is highly unethical to experiment on humans. However, if such agents were administered during pregnancy, children with gender variant behavior would be produced.
There have been some unfortunate accidents with some drugs administered during pregnancy. Some examples include the cyproterone acetate and other androgen blockers, diethylstilbestrol (DES), barbiturates, and dilantin.
Cyproterone acetate has been administered to women to control excess growth of hair. Sometimes, the drug has been taken by women who were pregnant. Genital abnormalities have been reported, and gender variant behavior. The same risk applies to other androgen blockers.
Diethylstilbestrol has been linked to not only abnormalities of the female reproductive system, but it has also been linked to gender identity problems in DES sons and abnormalities of the reproductive system.
Barbiiturate use during pregnancy has also been linked to gender identity problems in the children.
There are other drugs, including propecia, that are contraindicated during pregnancy because of they are considered teratogens. Teratogens do not always have a outward visible effect, they can also have an influence on how the brain develops.
