Citizenship, age and residency are the requirements for voting. All three proofs should be produced when you register for the first time. A National card should be issued which is good for life. Local residency laws etc will take care of the rest.
Of course, it will create a new forgery market.

A national ID will be ineffective and will meet with a huge amount of resistance from civil libertarians and conservatives. It will never pass.
If it does, I will do my best to be the first one to publicly burn it. I will not be stamped, bar-coded, tattooed, marked, scarred, or tagged in any other way by my government until my corpse sits lifeless and rotting.
