I just recreated almost all the text from one of the combat action reports and posted it HERE

Among other things, it included a damage report for PCF 94. It also says that, while 4 men other than Kerry were given treatment at USCGC Spencer, 2 others were also listed as injured that day. Kerry's injuries were listed as minor and he was returned to duty... 4 were medevaced from USCGC Spencer to 'destination unknown'.

Here's the damage reported to PCF 94:

          VRC-46 RADIO AND ALL REMOTE UNITS PILOT HOUSE INOP.
          AC WIRING SHORTED OUT.  ONAN GENERATOR INOP.  STEERAGE
          CONTROL AFTER HELM INOP.  STBD BILGE PUMP BROKEN.
          SCREWS CURLED AND CHIPPED.  RADAR GEAR BOX FROZEN.
          MAIN ENGINES EXPERIENCED DROP
ECHO:     BATTLE DAMAGE
GOLF:     A. (1) NO 2() YES (3) NO (4) NO (5) NO
HOTEL:    AN THOI, RVN

Is this the report where the boat designations were mixed up?
