Good question, I'm not sure why all three were documented on 8/12/69, but the injury dates are noted. Maybe somebody here on FR with knowledge of these things can answer your question

His medals were given AFTER he returned to New York?
(Awarded sure: sometimes the medal is received after the paperwork goes through ....
But this sounds like an "admiral's aide" writeup back home to get a photograph receiving medals from an admiral...
Who never knew him under real combat duty....
