You would think that this particular problem would have been discovered in the early stages of testing. However, switching from 6.8 mm to 5.56mm to save a few ouncues is a big mistake.

The U.S. Army has asked Congress for an extra $26 million to buy 7,000 XM-8 rifles next year. That sounds like almost $4K per rifle for some sort of an m16 replacement. I'm definitely in the wrong business. You can buy really fine rifles (Blasers) for that, and you can buy better military rifles for a hell of a lot less.
