The lever probably doesn't move during firing. Hope so. Even so, it looks like a bad place for the thumb.

Maybe the bolt doesn't blowback. Manual chambering of each round?

No, I don't think it was the same one, I could be (and frequently am) wrong though.
The article I saw was more along the line of "Hey, check out what this good'ol boy rigged up in garage!" as compared to a new product on the market.

I remember a photo with a caption mentioning the hundreds of balls of 00 shot that were in the air as it was fired full auto.

You are correct. "Sawed off" shotguns have been useful military weapons for a very long time. My older brother described how he cut off the barrels and stock of a side-by-side shotgun during his tour in Vietnam. The double barreled .12 gauge shot-pistol saved his life as he exited a tree line and ran smack dab into a VC. There wasn't much left of Charlie afterwards.

barrel under the barrel. A 10 shot six-shooter.

Even more curious, why on earth would you carefully aim a shotgun with a 7 3/4 inch barrel? The scatter on that thing is probably 10 feet at 10 yards.

I think it's a single shooter, manually cocked by the straight pull bolt...designed specifically to mount under a carbine...hence the left handed orientation of the bolt.

There is also a baton that can be put in the barrel...read about it in a special weapons magazine, supposed to be able to use it to bust a door off its hinges...

It's referred to as a 'straight pull bolt action', not a semiautomatic.

There have been a number of those.  The one I see the most, since it's used a lot in movies, is the Daewoo USAS-12

I like tube fed for manueverability, but usually you give up capacity to keep it from being too long.

I came across this one a couple of years ago and am really intrigued.  It's the Neostead, a South African design.  It's a bullpup, so it's compact, and they've set up twin tube magazines over the barrel.  You can even select which tube to draw from, so you can set up different types of ammo in each.

Ooog. What a nightmare.
The original "Master Key" concept proved that putting a shotgun underneath a functioning weapon WILL NOT enhance the weapon. In the end, you end up with ONE very bulky weapon that suffers in both roles.

The M203 differs in that you're AUGMENTING the capability of a weapon by adding a DIFFERENT capability. Putting a shotgun under an M4 is redundant. The best "breaching" weapon is the pistol-gripped Remington 870 with a short barrel. The Ranger Battalions have proven this years ago. Time of the R&D boys to start spending time in the field.

I believe there was an outfit that made a civilian legal (class 3 NFA?) 18" barreled shotgun attachment that went under a standard 20" AR-15 barrel. The attachment used the AR-15 receiver to get the 28" total length requirement. No, it wasn't the pump action "Crossfire" but a true independent attachment. Wish I had pursued it then so I would have one now. It would be the perfect set up for day long varminting.

What if the "short" shotgun is designated a pistol? marked, designated by the manufacturer as a "pistol"?

There is a derringer that is about the length of a 3 inch 410 shotgun shell that is legal. It fires the 45 long colt cartridges, or a 410 shot shell. It's not very accurate, but at close range it will put any man down, or at least make him wish he had been someplace else.
