FYI, the printed version just became available on Barnes & Nobel today Already added it to my wish list there. Will add it to wish list at Amazon as soon as that becomes possible. I'm under strict orders, from wife and daughters, not to actually buy any books until after Christmas. Hope to to get lots, including yours. What I don't get, I'll order myself on Dec. 25th, assuming I can get online that day, which might a problem, 'cause I'll be out in real fly over country, where our ISP is kinda thin on modems to call. But there's always the library, where I know they have public terminals.
