From an aviation safety viewpoint, these two get a pass. Out of the crew's control.

Excluding aircraft maintenance from aviation safety?

The UPS crash in particular was caused by the maintenance crews violating technical orders that were put in place after a prior fatal crash.

There is pilot error and other errors in aviation safety. Pilot lives we get a chance to examine what really happened. He dies, many times it is passed off as human error unless there is demonstrable catastrophic maintenance or aerodynamic destruction caused by structural or maintenance failures. First stop is always the crew.
