The original Out of Towners with Jack Lemmon.

“During the filming of The Out of Towners (1970), a manhole explosion scene went wrong, nearly injuring Jack Lemmon. The blast was stronger than expected, launching the cover high into the air, which landed near his head and hit his leg upon bouncing, IMDb reports. He stayed in character, and the real-life incident was used in the final film.”

Thanks for posting that scene with Jack Lemmon. He was a good actor to stay in character during the attack of the manhole cover.
