New images from airport security video shows the UPS plane's left engine detach from the wing moments before the deadly crash.

After initially blaming that crash on "pilot error", the final report said:
O'hare crash:
"use of a fork lift by maintenance workers had damaged the engine mount, or pylon,"
Pylon aft bulkhead attach point failed, same as AA crash
