Within the cycling community, this is called “getting doored.” It’s common enough that cyclist need always to be on the lookout for it. Mostly unintentional despite some egregious cases.

Most likely this woman suffered head injuries. (The falls I’ve had, I have always hit my head hard on the ground. Always had a helmet, though.)

Yes...if she wasn’t wearing a helmet, it’s quite possible for a head injury to be fatal.
