What was the blunder that caused it to sink?

What was the blunder that caused it to sink?


I would think at launch. When sliding sideways into the water it tipped too much, flooding. I’ve always wondered why we don’t hear more sinkings launching this way. But I’m not a biologist....eeerrrrr...I mean scientist!
