Presumably, because he had a twin. Some people believe Matthew is his twin, but there doesn't seem to be much evidence of this, so most likely his twin is unknown. The Acts of Thomas says he was called the twin because he looks so much like Jesus, and some groups claimed he actually was a twin of Jesus.

Why was Thomas called the Twin? Does anyone know?"Toma" is Aramaic for twin. Thomas was his "street name," just like Simon was called Rock (Peter), or Joseph was called Son of Comfort (Barnabas). We don't know Thomas' real name.
(PS: there was a Viking in France who took the name of Thomas, presumably when he was baptized. All of his descendants since have been known as the Thomas-sies, under variations such as Macy, Massey, Massie, and Massi.)
