Dear Charles,

As the parent of someone born in the .001% of people who are legitimately “trans”, here are some things you should know:

There are several very rare genetic disorders that can give rise to gender dysphoria. Gender dysphoria usually results from a congenital endocrine defect or some other anamoly that causes a disruption in the way the body produces or processes estrogen, testosterone, progesterone, or free floating adrenal androgens.

I watched it happen with my own child. When puberty hit, my beautiful baby girl began to morph naturally into an interesting looking boy. Her jaw flattened out, voice dropped, back spread out to a v, masculine body and facial hair, the works. Her system produced an inordinately high amount of free-floating adrenal androgens that her brain recognized as testosterone. Her body programmed her to develop as a male.

So, first things first: there must be an underlying medical condition for someone to be trans. Unlike the horsehockey that they are shoveling in school, gender is not mutable.

For girls, the conditions can include Congenital Adrenal Hyperplasia, Non-Classic Congenital Adrenal Hyperplasia (also known as late-onset CAH), Swyer’s Syndrome (where a person presents as a girl but is actually an incomplete male with XY chromosomes and a vagina), Turner’s Syndrome (possessing X/partial X chromosomes), PCOS, or a tumor on the pituitary gland.

Not everyone who has these conditions experiences gender dysphoria, and for those that do, only a small fraction will remain dysphoric past their early 20s when their endocrine system finally settles down. It is statistically impossible for all these young people to be trans.

It is time to take your grand-daughter to the doctor and run her through a battery of tests, including an ACTH stimulation test conducted by an endocrinologist. If she has one of these conditions, there are other health concerns to be worried about, too.

If there is no underlying medical condition, then it’s psychological and she needs a good therapist. She is not trans, she is brainwashed, confused, or acting out. Do not take her to a specialist in gender issues; they are all about affirmation and not finding the root cause of the problem. Do not affirm her chosen gender, do not call her by her new name, do not support her delusion. Do not let her go on hormones, support her surgery, etc.
