I read this as a shipbuilder discovered the faulty welds. So these are discovered by non original shipbuilder doing modification or repairs? The importance of competition?

Sole sourcing supply chains has been the bane of many manufacturing entities over the years. Regulatory capture is another, of course.

Documentation of work and inspections should be clear and fully investigated. I expect the story will come to incompetence followed by some "blind eyes" whether paid or unpaid.
