Nope more like a design/quality issue.

While it doesn’t sound like it’s an issue here, throttle by wire sucks......almost killed my neighbor a couple of years ago

In "the old days", if you reacted quickly, you could turn off they key switch, cutting off power to the ignition and fuel pump, shutting off the engine, allowing you to stop the car.

With today's push button start cars, it has to be in "park" to be able to shut it off.

What could possibly go wrong?

Throttle by cable never killed anyone!
The old holley double pumper carb with foot pedal/cable control worked very well for decades and still does!
