Here are the 3 possibilities concerning July 11th:

Planned hit on Trump, inside job all the way
Planned incompetence, lax procedure hoping that some idiot would take a shot at Trump ( even benign assistance given to the shooter ).
Structural incompetence, lax procedures hoping that someone would not take a shot at Trump.
