That is a man pretending to be a princess. She is a biological female who has done horrible things to her body. I'm not sure what her goal was, but the result is an abomination.

I have read she had severe infections after oral surgery, and operations to correct that resulted in her present appearance. Don’t know how true that is.

Sad part is , this was Princess Kalina of Bulgaria before...
She suffered some kind of dental infection which spread to other parts of her face - She had plastic surgeries (obviously failed) to reconstruct.
