I don't get it. The slide is inside the door inside the plane there is not way it could fall off unless the door was open in flight.

I guess it could malfunction inside the plane deploy and cause a real mess but it could not fall off.

Not all slides are aligned with doors. The "overwing" slides hatches are near the rear edge of the wing:
