The earliest (retroactively) diagnosed case in the world was, I believe, 1977 in Léopoldville, Republic of Congo (or Zaire).
The Hilary Koprowski trial along the Congo River was 1957. Not until some of the infected infant(s) where the SIV-1 mutated to HIV-1 grew to sexual maturity (early '70s) was it possible to kick-start the pandemic. It also takes years for AIDS to show symptoms.
The disease wasn't discovered until a string of rare cancers started to appear in the homosexual males of NYC, and shortly thereafter amongst homosexual males in L.A.
Adding one more thing: AIDs actually hit Haiti before the USA. Back in around 1979-1980 Haiti was a favorite debauchery destination for Manhattan homosexuals. There was a direct linkage of certain workers between Belgium and Haiti, traceable back to the polio vax trial in the Belgian Congo.
