It’s due to replication error.
You probably remember this from your introductory class in virology, where you learned that RNA viruses have a very high rate of replication error.
With Covid a successful error generates a variant with a spike protein different from what the vaccines are built on so your immune system doesn’t know to fight it. Really basic stuff. The same process at work in influenza.
