The woman sucked into the engine was a diversity hire which did not abide by the rules.

I suspect true same is true here.

People who ignore rules their entire lives and suffer no consequences for doing so should not be placed in positions where they can endanger other lives.

I am stating the obvious yet I don’t see that being practiced.
