One of my former bosses was on the accident investigation team for an Army Helicopter Crash.
Figured out that a bolt had been installed from the wrong side. When the control surfaces were exercised under load, the bolt hung and the helicopter pitched over and crashed, killing 4.
When the investigators finally discovered the cause, and then started questioning all people who would have touched that part, they found an old line worker who had been transferred into that particular job.
He said that bolts don’t go in from the bottom, they go in from the top: EVERYONE knows that. He ignored instructions and installed it the way they were supposed to be.
Killed four people.

I don’t understand your point. He did it correctly & that caused the crash?
