My guess would be cardiac arrest then a stroke which effected his nerve messaging between lungs and brain stem.

The most likely cause of neurological deficits is not a stroke but what we call anoxic encephalopathy. If deprived of oxygen long enough the brain cells die. While a stroke is localized to the area of the clotted vessel anoxic encephalopathy is more widespread
