No, he is making very large sums of money on very lucrative, numerous advertising 'opportunities' that arose the moment he went public. The more he trans, the more he earns. It's implied, not likely written down anywhere.

I stand corrected. I went to get a link from a YOUTUBE channel named "Odins Men' because I recalled it had a clip wherein the YOUTUBER estimated Dylan Mulvaney's monthly advertising/product placement income to be $70,000 per month. Comments I recall from that prior video were that Dylan's updates ("Day 12 of being a girl!") are not mere cell phone quality impromptu reports, but the posts by this trans-er are scripted and filmed in higher quality marketing videos. So I assumed his surgery was part of 'pay to play.'
I didn't find the clip I was looking for but found the same YOUTUBE channel (Odins Men) explaining that many states cover trans surgery because Obama made it a requirement for those medical facilities wishing to participate in Affordable Care Act registries. So it's likely Mulvaney makes the advertising money AND has his surgery covered by the Affordable Care Act. Dylan Mulvaney was immediately picked up by the Biden White House and was invited, had an interview there etc. Lots of money being thrown at this one.
With Dylan's financial windfall, another player has enterred the field. Another man with the same 'rah rah' trajectory reporting, as Dylan initially did with "Day 1 of being a girl'. Odins Men ( YOUTUBE channel) titled his video coverage of the 'new' advertising spokes-trans as " Knock-off Dylan is Worse than a Fake Rolex (hard not to laugh)." It's no accident this transer-of-the-hour has the same plastic, creepy central casting vibe and immediately begins the marketing by stating the brand name of his skirt, shoes etc.
But yes, as you've pointed out, when this horrible sham comes crashing down on post surgical (castrated) men, I imagine their decline will not be covered by anyone.
All of the kids who are being duped into these surgeries are going to feel awful when they have their genitals altered and realize that not only are they probably permanently incontinent, but that they will never be able to have an orgasm again.
