It likely is a maintenance issue, although a formation departure like that can lead to a wingtip vortex ingestion caused surge in the left engine, followed by the yaw/roll to the left that you see. It’s too low to recover so bam, into the grass he goes...

Just one of many possibilities.

You can see part of his wing tip come off just before the crash. He hit the other jet on take off. No chutes.
