The key here, to me, is that it is very likely people with higher BMIs have metabolic syndrome and glucose and insulin control issues/fatty liver. These are serious comorbidities, if you aren’t already on the healthy upturn, by going lower carb, exercising, sleeping well, etc. In those situations, metformin would already effectively be pretty useless, it would seem.

The real results are that both hospitalization and mortality are rare. “Due to the rarity of the hospitalization (2.0% overall) or mortality (0.2%)” It is a limited study with acknowledged flaws and contradicts existing valid studies, RCT or observational.

Not all Diabetics are obese or over weight at all.
There is a Genetic factor with some Diabetic patients.
