Everything in what you wrote was so identical to my situation, I could have written it.
Broke collarbone and had a 3rd degree separation when the front wheel departed. I had just lifted up on the handlebars to clear some uneven pavement I saw coming up at about 15 mph when the wheel came off. Truth be told, I think I had forgotten to tighten the handles.
