From the look of his hands he's had an IV bag hooked up to him. Who ever did it must have been trying hard to find a vein. That's not uncommon in elderly people as their skin is often very thin and frail and their veins are often hard to access.
My fathers hands looked a lot like that before he died. He was very ill in his final days.
