This is the part that infuriates me. This is a direct contravention of ADA rules and HIPAA rules. This is information that is confidential and only appropriate between patient and doctor. If you ignore them, refuse to answer them, or are vague or misleading in your answer, They will mark you in their book and non-vompliant.
Who knows what form the next visit will look like or entail. I am now convinced that the best course is to simply lie to them and tell them that I got the jab. If need be I will counterfeit a vaccination record.

Census enumerators and Census personnel are allowed to trespass, up to the moment the property owner asks them to leave.
After that, they're trespassing*. This Lake County Health Department bullplop is just that - bullplop. They're trespassing from the get-go.
*I was a Census supervisor in 2010, and an enumerator in 2020.
My daughter lives in a small village in Lake County. I don’t know about the rest of Lake County, but her village has a very low crime rate. Interestingly, they have a number of people from Eastern Bloc countries. I wonder how THEY will react.
