I know I pester you for your first-hand info but...

What is the cost of a monoclonal antibody treatment?
Is it counted that a person is “hospitalized” when receiving the transfusion?

You never pester...currently the monoclonals are being given for free. Due to some idiot bureaucrat I cannot administer it to an inpatient as it has been determined they do not meet criteria. I have discharged patients who are hospitalized but on the floor (not in the ICU) to get infused, and they have done beautifully.

There is no reason that a patient in the hospital who meet criteria (Positive test, symptomatic, and under 10 days from the onset of symptoms) should not be availed this cutting edge, life saving therapeutic that actually works.
