For COVID-19, that might look like "hypoxemia due to ARDS due to COVID-19". COVID-19 caused that death. That chain of events must only have direct links. In other words, you wouldn't put "hypoexmia due to COVID-19 due to cancer" because cancer doesn't cause COVID-19.
The comorbidities you're talking about are listed in Part 2. That section is for conditions which complicated survival, but were not part of the chain of events leading directly to death. So if a patient has diabetes or hypertension, those conditions are listed in Part 2. A person can live for decades with either of those, but if they get COVID-19, it escalates to ARDS, and they die from lack of oxygen, that's a COVID-19 death.
Further, if the wrong cause were simply listed for a bunch of deaths, deaths from those causes would go down and total deaths would remain the same. Except that deaths from heart disease, cancer, etc. didn't go down in 2020. Total deaths went up. By about 534,000. That's an over 18% jump from 2019. A normal year-over-year increase is 0.3% - 0.7%. So all the normal heart disease and cancer deaths were there, but suddenly there was a massive (World War II level) spike in deaths and it happened in a year when doctors and nurses around the world reported losing patients to a new disease.

There were so many deaths from covid in the US that I have gone since December 2019 to present and don't know a soul who has had covid or died from covid.

Last fall before the election, I drove to Arizona to visit a college buddy. News said don't go. Spike in new cases. Hospitals overflowing and temporary morgues being set up. My friend and I spent a week going out to eat Mexican food every day and didn't wear a mask once. No physical evidence of hospitals being flooded. No shortage of body bags. No bodies piled in the streets and alleys. It was all fear and hype by the government "experts".
I don't believe a word Fauci, the CDC, the WHO or any other quasi-government "expert" entity has said, even if my life depended on it. In my mid-70s, I've never worn a mask. Or followed any of the other made-up useless rules. I'm not getting the vaccine. According to what I read, I should have been dead by now.
I will die eventually but you can count on it not being from lung cancer or a car accident or heart disease. It will miraculously be from covid even though I have never taken a test to prove it otherwise.
