Underlying conditions. Most likely is vaccination did not confer her with immunity due to her underlying conditions.
If one’s immune system is not able to mount a response to either infection or vaccine, a vaccination will not confer immunity and an infection would be hard to fight off.

The article doesn’t tell us much other than a breakthrough rate of 0.04% (32 out of 71,371) for that Calif county with one fatality after ‘prolonged’ hospitalization of a patient with existing issues. From Napa’s statistics, their unvaccinated death rate (deaths/cases) is 0.08%, much higher than the vaccinated death rate of 1/71,371.
Vaccines are being reformulated on the fly for the mutants, (so we’ve been told) but originally targeted the Wuhan CCPVersion 1. The UK variant is the second most circulating in the USA followed closely now by the India variant with both being more aggressive that the original.
The problems in mRNA animal trials were always when challenged with the natural virus. I have seen no evidence that they overcame this deficiency.
We may very well be setting millions of ticking time bombs.
