Has anyone here successfully gotten a ‘jab’ without signing a release form?
My wife and I belong to the really big PPO on the West, and we remember signing the release form before getting the first shot.
We don’t remember signing anything for the second one besides having our temps check as we came in the building, like with the first shot.
The RNs giving us the second shot, entered the date on our cards, which we had to give to them before our 2nd shot.
We were in and back in our car in a total of 20 minutes, including the 15 minute waiting time after a shot.
My wife commented that we were halfway home before our 2nd shot was scheduled.
Yes. I did. That said, signing a release form means little if there isn’t ‘informed’ consent. It’s like signing a consent form before surgery/procedures but not being told about all the potential complications before you sign. It’s not valid unless the potential complications are listed on the release form.
