My CDC card has my first and last name, my date of birth, labels off both packages of Moderna vaccine, and the dates and initials of the persons who gave me my shots and initials of the place where given that match the vaccine package labels.
One label reads:

Manufacturer: Moderna TX, Inc.
Lot #....... exp. 9/3/2021
Site: FDOH Sarasota POD
where I use ....... to represent the seven-character lot number
