Dear Parents/Guardians and Students,

Many students and families will be going on vacations this summer and may have had difficulty scheduling students to receive a vaccination. I am sending this notification as an opportunity for any XXXXXX XXXXXX student (16 or older) who wishes to receive a Pfizer Covid-19 vaccination to participate in a Vaccination Clinic being sponsored by XXXXXX being held at XXXXXX XXXXXX School on May XX, 2021 in the gymnasium. At this time we know there is discussion of the Pfizer Vaccine being approved for students 12-15 in the near future, however, this clinic will remain available for students 16 or older only and will be restricted to only XXXXXX XXXXXX students. We are working to offer a follow up clinic to the XXXXXX XXXXXX Community later in the summer.

This is a voluntary opportunity for those individuals who want to be vaccinated. The second dose of the vaccine will be administered on the students' last day of June XX, 2021. Any senior student who wishes to participate should be available on June XX for the second dose. If any student is unavailable for the 2nd dose clinic, you should plan to have the second dose administered at another location vaccinating with the Pfizer vaccine.

Receipt of the vaccination is currently a personal and individual choice. If you would like to have your student scheduled for an appointment at this clinic, you should fill out the attached consent form and send it into school with your child (a consent form will be provided to students at school on Monday as well to bring home. The attached consent form must be filled out by a parent for any 16 and 17 year old student prior to receipt of the 1st dose of vaccine but will authorize consent for administration of both 1st and 2nd doses.

XXXXXX will need minimally 10-12 students to register in order to utilize XXXXXX XXXXXX as a host site for a vaccination clinic. If we do not receive interest to drive the clinic, XXXXXX will work with those individuals who did register to see where they may receive a vaccination.

Any interested student can be registered/scheduled by calling XXX-XXX-XXXX. If your call goes to voicemail, please leave the name of the student being scheduled and the preferred callback time to complete the process.

This message is not an endorsement of receiving a vaccination or of any one vaccine over another. Administration of the vaccine is sponsored by and provided through XXXXXX. Neither XXXXXX XXXXXX or the Diocese of XXXXXX has authority to approve or administer vaccinations to students. It is recommended that you consult your primary physician before making any decisions regarding vaccinations or your general health.

Attached to this email: Consent Form and Pfizer Fact Sheet. XXXXXX does not utilize the information provided for transfer to any individuals' general practitioner. You will not receive any corresponding solicitation from XXXXXX.
